The only explanation is if the configuration was changed in the meantime. The behaviour can’t change on it’s own, that is not possible.
Usually the reason for these strange behaviours is a layer of cache somewhere. It is possible that your CDN, browser or other cache stored a previous output on some page and it got messed up. Then after refreshing and saving the configuration it may have been cleared and it does not appear anymore.
