I copied one of my search forms to a new form, but the new form looks completely different and doesn’t work. You can see the issue on my test page at this URL https://joespinoza.com/test/

I tried both search bars but they seem to work all right on my end:
– Form 4: https://i.imgur.com/ZoBqDsi.png
– Form 5: https://i.imgur.com/kQTBAeq.png
Make sure to clear your site and browser cache, then it should work all right.
