Do related posts display only via shortcode. If so, I don’t want to have to add it to every post.
If I don’t use the shortcode the related posts to not appear.
What am I missing?

The issue was, that you had the ‘fill’ feature disabled, so that the plugin was not displaying anything when there were no related post matches. The override feature was also enabled, but nothing was defined for the override. I have corrected these options, now you should see the plugin under the post contents: https://i.imgur.com/GIpfjyj.png
