Hello,
The website is about online doctor appointment.
Search will be by speciality, by city and by name of physician.
For that, I created three instances: provider_specialty_title, provider_city_title and provider_doctor title. I also created the same in database usermeta table. Shortcodes where automatically created. Is it correct what I did? How do I display the search box on website front page?

To display the search, use the search shortcode, wherever you need it: https://goo.gl/RhDR0F
The database looks okay to me. Just enter the meta field names you want to search on the General Options -> User search panel, like so: https://i.imgur.com/gmM3z9U.png
