I added a search to our widget however it comes up incorrectly, like screenshot here:
I have everything configured correctly from what I can tell.
We added you as a WP admin.

It is very likely due to site/asset cache, on the screenshot the search style is missing from the page. If you have any cache plugins enabled, make sure to clear their cache, as well as the browser cache. (by reloading the page a few times by hitting CTRL+ SHIFT + R).
