Search Works in Preview, but Not on Website

Hi, I recently downloaded your plugin for codecanyon. For some reason, the plugin works on the preview (http://imgur.com/a/wPJfq), but when I try to put it in my theme it comes up like this http://imgur.com/a/DXUOS and the search doesn’t work. Please get back to me when you can.
